What is a DOT physical?

A Department of Transportation (DOT) physical examination must be conducted by a licensed “medical examiner” listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry.

Why do I need a DOT physical?

For commercial motor vehicle (CMV) drivers, the most important safety feature is YOU – the driver! Each time you turn the key, you are responsible for your own safety, as well as the safety of all the people who share the road with you. The physical examination you take for your medical certificate confirms that you are healthy enough to safely perform the demanding job of a CMV driver and keep our Nation’s roads safe.

What do I have to bring with me for the office visit ?

Must bring a Photo identification (Commercial Driver’s License if you’ve one)

If you have any medical history of diabetes, heart disease, high blood pressure, sleep apnea, depression, anxiety, currently taking high risk medications like opioids (oxycodone, Tramadol, Hydrocodone, etc.), benzodiazepines (Xanax, Clonipine, alprazolam, clonazepam, etc.) or take other prescription medications, please bring your current medication list, the name and office location of the prescribing physician/provider.

What to expect on the day of the office visit ?

A DOT exam is similar to an annual physical exam with emphasis on certain exam components. You’ll also get your height and weight measured, vision, hearing, blood pressure, pulse evaluated and have a urine test in addition to a physical exam.

Remember to bring eye glasses and /or hearing aids if you use one.

How do I receive my certificate ?

You’ll be given a paper print out of the certificate if you are certified. This certificate will be electronically reported to FMCSA and a copy will be faxed to Arizona DOT when applicable.

How long my certification will be valid for ?

A DOT physical exam is valid for up to 24 months for healthy drivers. The medical examiner may also issue a medical examiner’s certificate for less than 12 months or less when it is desirable to monitor a condition, such as high blood pressure, heart disease, diabetes, depression, anxiety, sleep apnea, etc.
